Artificial intelligence has never been more accessible. Now that every man and his dog can ping OpenAI’s API for pennies, the gap between idea and compelling prototype is next to zero. Yet, despite the enthusiasm for AI among businesses, industry research consistently shows that 80–90% of AI pilots never make it into production (RAND). So why is that? Well, the problem isn’t with the technology, it’s with how we’re using it.

In this article, taken from insights from a recent webinar I did with Andrei Papancea, CEO, NLX, we’ll look at why so many AI initiatives fall short of their promise. I’ll draw on both mine and Andrei’s field experience, real-world patterns and practical insights. We’ll explore the most common reasons AI projects fail to reach production and, more importantly, what to do about it. You’ll walk away with a clear, actionable playbook for designing, building and scaling AI solutions that don’t just impress in the boardroom but deliver results in the real world.

The 8 real reasons AI projects fail

Understanding the common failure points is the first step to overcoming them. AI projects often fail not because of technical limitations, but due to poor alignment with business needs and weak implementation processes. Here are the main reasons:

1. Solving the wrong problem

Many AI initiatives target issues that are too small, already solved through existing channels or lack business urgency. This leads to solutions that may function technically, but don’t justify continued investment.

I’ve had this experience myself a few years ago. We worked on a voice AI project for a client that already had a compelling self service journey online. They were keen to prove the feasibility of doing the same over the phone. The project was challenging, but we validated that it was feasible, but it never went to production because the cost of sustaining the solution was expensive considering there was already an online alternative.

Instead of aiming for novelty or low-hanging fruit, teams must tackle meaningful, measurable problems that are aligned with strategic goals.

2. Starting with technology instead of need or design

Too often, teams jump straight to tools like ChatGPT or Microsoft Copilot without first understanding the problem, user journey or what a successful experience looks like. This “solution-first” approach results in poorly scoped projects, disjointed experiences and limited business value.

You end up ‘doing AI’ for the sake of doing AI. A problem that’s rife at the moment and that I discussed in more detail with Sonia Ingram, AI Director, Pandora, on a recent VUX World podcast.

If you diligently design the ideal experience, you can map out the specific capabilities that will make the experience great. Instead, many businesses and developers are starting from the point of ‘how can I make an AI agent for this’.

3. Lack of process understanding

You can’t automate or augment a business process you don’t fully understand. Many projects underestimate the complexity of legacy workflows, undocumented tribal knowledge and edge cases. Without a clear and deep understanding of the current-state process, AI implementations will be brittle or misguided.

It’s easy to think that AI will figure it out if we explain what we know in a prompt, but proper business process mapping and capability assessments will help you identify your actual requirements, as well as where the waste is. This gives you the opportunity to potentially redesign that process with AI, rather than trying to throw AI over the top of a broken process. I call the latter the ‘lipstick on a pig’ problem.

4. Disconnected from real-world systems and data

Even the smartest AI can’t act if it’s not integrated with operational systems. Projects often fail when they stop at natural language understanding without connecting to the APIs, databases and workflows required to actually complete tasks. A chatbot without execution power is just a glorified search box.

I’ve had numerous projects stall due to a lack of API access, so you need to catch this up front to prevent you from starting something you can’t finish.

5. Overconfidence from easy prototyping

The rise of low-code tools and large language models has made building demos deceptively easy. But a slick prototype is not a product. Many teams mistake a quick 10-minute prototype for a production system, overlooking the 90% of work required to scale, secure and operationalise that solution.

In the same way as misunderstanding agile project management gave some companies and people a licence to skip documentation and pretty much make things up as they go along, generative AI has had the same influence on solution development. Builders seem to have amnesia and have forgotten that they’re building enterprise-grade applications. They’re skipping any kind of discovery, solution design, testing and deployment planning and jumping straight to building whatever they can in whatever way they choose with LLMs.

6. Confusing Prototypes, POCs, MVPs and Pilots

Muddled definitions lead to mismatched expectations and misaligned work. Many AI projects fail because they’re wrongly positioned as more advanced in the development cycle than they are. What should be called a prototype to validate design feasibility is wrongly called an MVP and expected to be launched in 3 weeks.This is because many confuse the stages of product development and work on the wrong aims.

  • A prototype should test design assumptions
  • A Proof-of-Concept (POC) should test whether something is technically feasible
  • A Minimum Viable Product (MVP) is the first version of usable product that has only the minimum amount of features it needs to meet the needs of most users
  • And a Pilot is a project phase, not a solution, within which a small-scale deployment of your MVP is launched to a segment of customer for feedback.

Deploying a prototype as a pilot, or calling a POC an MVP creates chaos for stakeholders and misaligned expectations for users. It’s no wonder you can’t get out of pilot, when you’re taking a proof-of-concept into it.

7. Failure to define and measure success

If you don’t define KPIs up front, you can’t prove ROI later. Projects without clear success criteria and tracking mechanisms often flounder after launch. Teams need to align on what success looks like, how to measure it, and what thresholds will trigger scaling or stopping.This isn’t easy to do in a world where a black box can be responsible for application logic, and so granular analytics might be harder to put together than before. However, if you don’t understand what numbers to reach to have a solution that can be considered as ‘working’, then what on earth are you doing?

8. Treating launch as the finish line

Launching an AI system isn’t the end, it’s the beginning. Without an iteration plan, support structure, and feedback loop, the system will quickly become outdated or fail to adapt to new needs. Continuous learning and improvement must be built into the project from day one.

By recognising these pitfalls, teams can adopt a more intentional and structured approach to AI delivery and severely reduce the risk of failure.

12 ways to make your AI project a success

Making AI projects a success, in some ways, is the inverse of the above. However, there are a few more tips you can take on board to make sure you’re successful.

1. Start with a meaty business problem

AI initiatives should always begin by identifying a substantial, high-impact problem. These are problems that, if solved even partially, will deliver tangible ROI.

A great AI project starts with identifying a problem that’s either costing the business significantly or impeding growth and one that leadership cares deeply about solving.

This is the most important step of all. Toys are fun, but outcomes are required.

2. Avoid ‘Shiny Object Syndrome’

It’s tempting to start with the solution, especially with generative AI grabbing headlines. But successful AI projects don’t begin with “let’s use AI here.” They begin with a business challenge, customer pain point or strategic goal, followed by solution design. Tech comes later.

Falling in love with the solution before defining the problem often results in poor fit, low adoption and wasted effort. Leaders need to temper the hype and ask, “What outcome do we want?” and then, “Is AI the best way to get there?”

I fully appreciate that this is easier said than done when you have the CEO telling you that they want to be seen to be using AI, but you have to manage expectations and defend the process. You might well end up with that generative AI solution in the end, and if you do, you’ll know that it’s absolutely the best possible solution for your problem.

3. Understand the business process deeply

You can’t automate what you don’t understand. Many failures stem from a superficial understanding of the business process being automated. Processes may involve edge cases, undocumented exceptions or dependencies buried in legacy systems.

Spend time mapping out the full workflow. Engage process owners, customer-facing teams and IT stakeholders. Capture the nuances and decision points that aren’t always visible from documentation. A well-understood process is the backbone of a well-designed AI solution.

It’s tempting to think that LLMs will handle everything if you can just write it down, and it’ll fill in the blanks. This is folly in the vast majority of cases and can induce laziness. You want to map your processes in detail first. Then, you have a much better chance of understanding how to improve, streamline or automate (even a part) of it.

4. Ensure systems and data readiness

AI doesn’t exist in a vacuum. It must interact with other systems: CRMs, ERPs, ticketing platforms, databases. These integrations are where many projects falter.

Ensure you have access to the necessary APIs, data and authentication mechanisms. Additionally, assess data quality and availability. Do you have the data needed to train or support the model? Are there regulatory or security constraints?

A data readiness assessment before you build your POC is typically a good idea and will save you wasted effort.

5. Design the ideal experience first (independent of tech)

Before touching code or selecting vendors, sketch out the ideal user experience. What should the journey look like? What should users see, hear or feel at each step? This approach helps anchor the project in user value and prevents the design from being dictated by platform constraints.

It also helps you think creatively; imagining solutions that don’t yet exist and evaluating whether AI is truly needed. Designing independently of technology ensures you’re solving the right problem in the right way.

You might think ‘well doesn’t this limit the value of large language models’? This of course depends on the use case. Most use cases require the description of the thing that you want the models to do. It’s mostly business rules explained in words, rather than coded logic. In most cases, AI developers are already explicitly dictating what the models should do. What we’re suggesting here is that by first understanding what you intend the experience to be like, then if LLMs are part of the solution, at least you now have a) material for your prompts and b) test criteria.

6. Get to an end-to-end ‘Hello World’ quickly
Don’t aim for perfection out of the gate. Instead, focus on creating a thin slice of the full solution that demonstrates the entire flow, from user input to backend action. This is what we used to call ‘the happy path’. This is your Prototype.

This could mean a single use case with mocked data or limited functionality. The goal is to validate design decisions, experience integrity and identify gaps early. You’ll uncover integration points, UI components and orchestration requirements. Getting an end-to-end prototype up quickly builds momentum and helps clarify technical and business assumptions. It then sets you up for your Proof-of-Concept.

7. Validate design before scaling
Once the experience is defined and a prototype is in place, test it rigorously. This might mean usability testing, sandbox demos with stakeholders, or internal pilots. Use this phase to confirm that the user journey is intuitive and the system behaves as expected. Capture feedback early and often. It’s much easier (and cheaper) to adjust at this stage than after full implementation. Remember: a validated design is a scalable design.

The hardest part of validating prototypes or even a POC, is managing expectations. People have expectations that you can’t control. You have to be very clear with your stakeholders about what stage your project is at and what you’re aiming to learn. If you’re testing a prototype, be clear with those who test it that all you’re aiming to do is understand whether the experience is on the right lines. POC, you’re validating that it’s technically feasible and that ‘it works’.

If your stakeholders don’t understand what stage you’re at and your specific learning goals, they’re going to think that the solution is poor or give you a big list of features that they expect, rather than what’s actually needed.

8. Define KPIs before building
Success must be defined upfront. Collaborate with business stakeholders to agree on the metrics that will determine whether the project is successful. These could include automation rates, error reduction, cost savings, customer satisfaction or task completion time.

Having these metrics in place ensures everyone is aligned on the goal and provides a framework for iteration and decision-making post-launch.

9. Make sure you can track the right metrics in production
During the build of your MVP, you have to be able to actually track those KPIs, otherwise, it’s pointless having them.

Once the solution is live, monitor performance continuously. Don’t just track technical uptime or response times. Monitor business impact; how often are tasks completed? Where do users drop off? Are outcomes improving over time?

Implement detailed analytics and dashboards to give product, design, and engineering teams visibility into what’s working and what’s not. Without measurement, you can’t optimise.

10. Adopt true agile practices
AI projects benefit from being broken down into manageable, testable components. Embrace the agile mindset of continuous delivery and feedback. Release early and often. Use retrospectives to improve your process. Work in cross-functional teams that include business stakeholders, designers, and engineers.This keeps everyone aligned and ensures that adjustments can be made quickly based on what you learn.

11. Be willing to kill or pivot projects
It’s better to stop an unviable project early than let it drain resources for months. If discovery reveals the problem isn’t substantial, the data isn’t there, or the systems won’t support it: stop.

Likewise, be willing to pivot. Your initial hypothesis may be wrong. That’s okay. Use what you’ve learned to adjust the direction and focus on delivering value, not salvaging sunk costs.

Eric Reis in The Lean Startup refers to having a solid Vision, a Strategy that can pivot, and a Solution that’s continuously optimised. If you’re hitting roadblocks, you might not need to scrap the solution, but perhaps pivot the strategy.

Going back to the failed case I referred to earlier, if we’d have pivoted our strategy and optimised our solution so that the voice solution simply directed users to the existing self-service options, or used something like Voice+, then I suspect we’d have had more success.

12. Enable post-launch iteration
The launch is just the beginning. No matter how well-tested your solution is, users will behave in unexpected ways. If you expect your pilot to solve 100% of user needs on day 1, you’ve misunderstood the purpose of a pilot and the underpinning MVP.

Expect continuous iteration once you’re out the door. You now have a digital employee working 24/7 for your business and you need to constantly train and improve them based on feedback from the real world.If you don’t optimise, you’ll inevitably believe that the solution doesn’t do what you intend it to do and deem your pilot a failure.

Putting it into practice

AI has the potential to transform the way businesses operate; streamlining processes, enhancing customer experiences and unlocking entirely new capabilities. But potential means little without execution. As we’ve seen, the reasons AI projects fail are rarely technical. They’re strategic, organisational and procedural. The good news? These pitfalls are avoidable with the right mindset, structure and approach.

By understanding the 8 real reasons AI projects stall and applying the 12 practical recommendations we’ve laid out, your team can flip the script. Instead of building impressive demos that gather dust, you’ll create systems that solve real problems, scale with confidence and deliver long-term value.

The organisations that win with AI won’t be the ones chasing shiny objects – they’ll be the ones who design with purpose, validate with discipline and execute with excellence.
